Residential Care Homes
Our Intermediate Care Facilities for the
Developmentally Disabled - Handicapped (ICF-DDH) are
Licensed by the California Department of Health Services
and designed to provide 24 hour care, supervision and
support to individuals who have a recurring but
intermittent need for skilled nursing services.
Owned and operated by Serra Center and located within
the residential community of Fremont, Serra Center
operates four ICF-DDH homes. Significant thought and
care have been given to create a warm and welcoming
home environment in each of our uniquely different
homes. From the color of the walls, to the furniture
and decorating, to an accessible and tasteful floor
plan, we have created an environment in which each of
the 6 individuals residing within a home is comfortable
and is provided opportunities for the training,
recreation and socialization of their choice.
Our dedicated team of nurses, physicians and program
professionals work directly with each individual and
their family to develop a personal care plan that
focuses on each individual’s medical and support care
needs, as well as facilitating individual choice, goals
and dreams.
With over 30 years of experience and community
